Product listing is conducted through the FDA’s Cosmetic Direct Portal. To ensure compliance, the following information must be included in your cosmetic product listing:
- Facility Registration Number: The registration number of each facility where the cosmetic product is manufactured or processed.
- Responsible Person Details: The name and contact number of the responsible person, along with the product name as it appears on the label.
- Cosmetic Category: The applicable cosmetic category or categories for the product.
- Ingredients List: A complete list of ingredients, including any fragrances, flavors, or colors, with each ingredient identified by the required or common name.
- Product Listing Number: If previously assigned.
- Submission Type: Whether the submission is initial, an update to content (annual), or an abbreviated renewal.
Additional Information Requested by the FDA:
- Parent Company Name: If applicable.
- Type of Business: As listed on the label, i.e., manufacturer, packer, or distributor.
- Label Image: Currently, JPG files are accepted.
- Product Webpage Link: URL to the product’s webpage.
- Professional Use: Indicate whether the product is for professional use only.
- Responsible Person DUNS Number: For the address listed on the product label.
- Unique Ingredient Identifiers (UNIIs): If applicable.
- Additional Contact Information: For individuals associated with the listing.
